QLogic offers a wide range of benefits that address the diverse needs of employees and their families. We provide comprehensive benefit programs designed to help employees maintain good health and plan for retirement. As part of our program, we offer flexible plans that allow employees to choose the benefits that best meet their personal needs. Most programs are effective the first day of hire.
Medical Plans QLogic offers a comprehensive top-tier medical program pre-bundled with additional benefits (see below). Several options are available. Depending on work location, employees may choose from:
- Preferred Provider Organization (PPO) plan
- Health Maintenance Organization (HM0) plan
- Point of Service (POS) plan
- Traditional Indemnity plan
When enrolled in a medical plan, the following are automatically included:
- Vision Plan
- Prescription Drug Program
- Employee Assistance Program
Dental Plans QLogic offers a comprehensive dental program that covers preventative care, basic and major services and orthodontia.
Life and AD&D Insurance QLogic provides company paid Life and AD&D insurance to all regular, full-time employees equal to one times the employee’s annual salary. In addition, employees have the option of purchasing Supplemental Life and AD&D insurance as well as Dependent Life insurance coverage at minimal cost.
Short Term Disability For periods of short-term disability (less than 90 days), QLogic employees are assured continued income, provided eligibility requirements have been met.
Long Term Disability QLogic provides its employees with Long Term Disability Insurance, which protects against the loss of regular income during periods of extended disability.
Flexible Spending Accounts Health Care and Dependent Care Reimbursement Accounts are available to allow employees to set aside money on a pre-tax basis to cover eligible expenses.
Retirement Benefits Employees are eligible to participate in the QLogic Retirement Savings Plan (401(k)).
Incentive Program All active employees, except those on a commission plan, are eligible to participate in the QLogic Incentive Plan. This plan provides cash incentive at the end of each fiscal year based on company and individual performance.
Employee Stock Purchase Plan Employees may participate in the QLogic Employee Stock Purchase Plan which allows the purchase of QLogic stock at a 15% discount via payroll deduction. Employees are eligible at the first enrollment period following their date of hire.
Personal Time Off (PTO) QLogic recognizes the value of time off for vacation, personal business, sick days, religious holidays or emergencies. PTO provides employees with the flexibility to manage their time off from work based on individual needs. Employees are eligible to take PTO after a minimum accrual. Hours accrue on a pro-rated basis from the day of hire.
Holidays *QLogic observes 11 company-paid holidays per year, including a shutdown over the December holidays.
Other Benefit Programs Include:
Education Assistance QLogic provides educational assistance for full-time employees for pre-approved job or degree related classes. Reimbursement includes tuition, registration, books and lab expenses.
Employee Referral Program Cash rewards are paid to eligible employees who submit qualified referrals that are hired by QLogic, and fulfill program requirements.
Patent Program QLogic encourages and rewards contributions that protect the company’s intellectual property. Cash and non-monetary rewards are awarded for application filing as well as patent grant.
Personal Computer Purchase Program Employees may purchase computer systems for home use, with up to two years interest free via payroll deductions.
“Q” Difference QLogic plans a variety of employee activities and events throughout the year. Discounted tickets to a variety of attractions and sporting events are also available.
Wellness Program QLogic has established a Wellness Program that includes reimbursement for a portion of health club membership fees.
